It is also best to call your insurance provider before your appointment, to confirm which of our services are covered. If you wish to cancel or reschedule your appointment, please call our office, 24 hours in advance, in order to avoid a $25 cancellation fee.

If you need a referral for your insurance provider, please contact your Primary Care Physician and ask them to fax a referral to us before your appointment.

We accept and participate in most insurances plans. If your physician is out of network with your plan, payment in full is expected at each visit. It is important to note that even if the physician has a contract with an insurance company selling insurance plans on a health care exchange, those plans may possibly not include the physician as an in – network provider and the patient may not have out-of-network benefits.

If we receive notice from your insurance company after a service is performed, it is your responsibility to pay the ‘out of network’ amount owed according to your insurance plan. Please contact your insurer with any questions you may have regarding your coverage to receive the maximum benefit.
